Wash Sales, ISOs, & "Consider Your Options"

On the page for Wash Sales, there is a reference to Consider Your Options:

"As explained in my book, Consider Your Options, a wash sale involving shares of stock acquired through an incentive stock option can be a planning disaster."

I can't find the section that addresses this. The index only points to pg 52, which only addresses the rule generally. Is a wash sale considered the same as using stock to exercise an ISO (Chapter 21)? I don't think this is the case, but I can't find a single mention of Wash Sales together with ISOs anywhere in the book.

Could somebody maybe point out the page numbers for me?

We'll make a note to add index entries making this information easier to find. See chapter 15 under the heading "Other early dispositions" beginning on page 138 and chapter 19 under the heading "Bad bail-outs" beginning on page 172 (page numbers refer to the 2014 edition).
