Zero Net Pay - Medical Premium Question

Daughter is part-time school teacher. Husband lost job. Added husband and 2 kids to her Medical and Dental insurance.

Not enough salary to cover it all, so she will get $0 net pay and send them a check for $500 to makeup the difference each month.

ON her W-2 would the state report the full premium paid (deductions + the additional premium payment she makes)? Or will they report only the deductions?

If they only report the deductions, how would we report the additional premium pmts she will make for income tax filing? I'm hoping it would be treated like an additional pre-tax amount and not subject to the 10% medical expense threshhold
