Ordering a Social Security Statement

I'm trying to help someone order a social security statement via the mail - if you recall it's the kind they used to send out to all of our homes automatically in 2010. It includes earnings record and estimated benefits, etc. It's called Your Social Security Statement - or used to be. It's for someone in their '50s who is not on SS.

I can't find the request form he'd need to fill out and mail to the gov in order to get the statement.

I've been looking for a long time. Is someone willing to provide me with the exact link?

Is this the proper one?[www.ssa.gov]

Looks like it to me.... [secure.ssa.gov]

doesn't show the form but if you google the form #, you get that same form.

However, it's pretty easy for the earner to set up "my account" and just print it off.
